Medication management that keeps every dose safe.

Timely reminders, organized pills, coordinated refills, and clinical oversight from Dr. Okon's team — so the right medication is taken the right way, every day. Trusted by Houston families managing complex medication routines.

For someone managing several prescriptions, medication is one of the riskiest parts of daily life — and one of the most fixable. At Ama Ultimate Home Health Services, medication management brings structure, vigilance, and clinical accountability into the home, so families can stop worrying about missed pills and start trusting that every dose is handled right. Our care is led by Dr. Amaette Okon, DNP, who brings more than two decades of clinical experience to every plan.

Why medication management matters

Medication mistakes at home are far more common than most families realize. A skipped dose can let a condition flare; a double dose can be dangerous. When someone takes five, ten, or more medications, the odds of a harmful drug interaction climb quickly — and the consequences can be severe.

Among seniors taking multiple medications, these errors are a leading cause of avoidable emergency-room visits and hospital readmissions. A confusing schedule, fading memory, or a single new prescription added without review can quietly put someone at real risk. Steady oversight turns that risk into routine.

What we do

Every plan is built around the individual, but our medication management services commonly include:

  • Timely, consistent medication reminders
  • Pill organizers and coordination of refills
  • Watching for side effects and dangerous interactions
  • Syncing the medication plan with doctors and pharmacies
  • Clear documentation of what was taken and when
  • Nurse-practitioner oversight by Dr. Okon's clinical team

That oversight is the difference. Reminders alone help, but a clinically supervised plan catches the problems reminders never could — the duplicate prescription, the interaction, the side effect that needs a doctor's attention now rather than next month.

The clinician-led advantage

Because Dr. Okon is dual-certified as both a Family Nurse Practitioner and a Psychiatric Mental Health Nurse Practitioner, our team can coordinate psychiatric and physical medications together — a combination that often trips up fragmented care. Mood, sleep, and behavioral medications interact with everyday prescriptions, and managing them under one clinical eye keeps the whole regimen safe and aligned. Who benefits

Medication management is a fit for seniors juggling multiple prescriptions, individuals living with dementia or memory loss who can't reliably track a schedule, and clients recently discharged from the hospital with new or changed medications. It pairs naturally with our personal care and is a cornerstone of safe post-surgery recovery at home. If you're unsure where to start, reach out for a free assessment.

Frequently asked questions

Can caregivers give medications, or just remind?

It depends on the level of care. Trained caregivers provide reminders, set up pill organizers, and help clients take medications they self-administer. When hands-on administration or clinical oversight is needed, Dr. Okon's nurse-practitioner team directs and supervises the plan within the scope allowed by Texas regulations.

How do you prevent dangerous drug interactions?

We keep a complete, current medication list and review every new prescription against it. Our clinical team watches for duplicate therapies, harmful combinations, and side effects, and syncs directly with your doctors and pharmacy so changes are caught before they cause harm.

Is medication management covered by Medicaid?

For qualifying Texans, medication support is often included as part of a covered home-care plan through Traditional Medicaid and managed-care plans such as Molina, Community Health Choice, and UnitedHealthcare. We confirm eligibility and handle the paperwork at no charge.
